Eligibility for Hawthorn House

Hawthorn House is an assisted living facility (ALF) designed to provide personal care services in the least restrictive and most home-like environment. Hawthorn House offers a variety of personal and nursing services designed specifically to meet an individual's personal needs.

The Hawthorn House also offers a memory support wing which features a safe and secure environment, optimized to better care for those with progressive cognitive impairments such as Alzheimer’s, dementia, or other memory conditions.

Our dedicated memory support staff provides supervision 24 hours a day, assist the residents in all aspects of their activities of daily living, and offer fun, engaging daily activities intended to stimulate memory and physical exercise.



An individual must be age 55 or older and meet the following minimum criteria to be a resident at Hawthorn House:

  • surviving spouse of a military retiree

  • military couple

  • dependent of an active-duty military member

  • military retiree


Physical and Medical Considerations
  • Must be free of a communicable disease, which is likely to be transmitted to other residents or staff. Exception: A person who is Human Immunodeficiency Virus (HIV) positive may be admitted if otherwise eligible.

  • Must be able to perform every day, basic activities necessary to daily life with assistance or supervision.

  • Must be able to transfer with assistance, if necessary.

  • Must have dietary needs that can be met by the facility.

  • Must not be a danger to self or others.

  • Must not be bedridden.

  • Must not have Stage 2, 3 or 4 pressure sores.

  • Must not require 24-hour skilled nursing services or professional mental health care.

  • Must be deemed appropriate for admission by the Admissions Board and meet all other eligibility requirements.

Financial Assistance

The Air Force Enlisted Village offers a Benevolence Program that is graciously funded by active duty and retired Air Force personnel during the annual Air Force Assistance Fund.


Surviving spouses of retired enlisted Air Force members may be eligible for our Benevolence Program and receive subsidized monthly service fees. Some of the criteria may be waived for admission in dire need situations. Please contact our Admissions Coordinator at 850-609-2550 for more information.


Some residents may qualify for VA Aid and Attendance. Hawthorn House does not accept Medicare/Medicaid.
