Eligibility for Hawthorn House

Hawthorn House is an assisted living facility (ALF) designed to provide personal care services in the least restrictive and most home-like environment. Hawthorn House offers a variety of personal and nursing services designed specifically to meet an individual's personal needs.

Hawthorn House also offers a memory support wing which features a safe and secure environment, optimized to better care for those with progressive cognitive impairments such as Alzheimer’s, dementia, or other memory conditions.

 

Our dedicated memory support staff provides supervision 24 hours a day, assist the residents in all aspects of their activities of daily living, and offer fun, engaging daily activities intended to stimulate memory and physical exercise.

 

Eligibility
  • All veterans and their families, 55 and older.

 

Physical and Medical Considerations
  • Be free from signs and symptoms of any communicable disease that is likely to be transmitted to other residents or staff. An individual who has human immunodeficiency virus (HIV) infection may be admitted, provided that the individual would otherwise be eligible for admission according to this rule.

  • Be able to perform the activities of daily living, with supervision or assistance if necessary.

  • Be able to transfer, with assistance if necessary. The assistance of more than one person is permitted.

  • Be capable of taking medication, by either self-administration, assistance with self-administration, or administration of medication.

  • Not have any special dietary needs.

  • Not be a danger to self or others as determined by a health care practitioner.

  • Not require 24-hour licensed professional mental health treatment.

  • Not be bedridden unless the resident is receiving licensed hospice services.

  • Not have any stage 3 or 4 pressure sores.

Financial Assistance

The Air Force Enlisted Village offers a Benevolence Program that is graciously funded by active duty and retired Air Force personnel during the annual Air Force Assistance Fund.

 

Surviving spouses of retired enlisted Air Force members may be eligible for our Benevolence Program and receive subsidized monthly service fees. Some of the criteria may be waived for admission in dire need situations. Please contact our Sales Coordinator at 850-609-2550 for more information.

 

Some residents may qualify for VA Aid and Attendance, we do not process the request for VA Aid and Attendance, but we can provide some information and guidance. Hawthorn House does not accept Medicare/Medicaid.
